Master Social Worker Opportunity

We are seeking a skilled Master Social Worker to join our team at Innovative Renal Care. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Responsibilities:
  • Assess patient needs and develop personalized care plans
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care
  • Provide education and guidance on disease management and treatment options
  • Conduct regular assessments and evaluations to ensure patient progress and adjust care plans as needed
Requirements:
  • Master's degree in Social Work from an accredited institution
  • Current state licensure (e.g. LCSW, LMSW) and meet all state regulations for practice in a dialysis setting
  • Two years of clinical experience, preferably in an ESRD setting
  • Excellent communication and interpersonal skills
