Key Responsibilities- Patient Assessment and Education: Conduct thorough assessments of patients' physical, emotional, and financial needs, and provide education on disease management and treatment options.
- Care Planning and Coordination: Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans that address patients' unique needs and goals.
- Communication and Interpersonal Skills: Demonstrate excellent communication and interpersonal skills to build strong relationships with patients, families, and healthcare professionals.
- Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, assessments, and care plans, and provide regular reports to healthcare teams and management.
- Education: Master's degree in Social Work (MSW) from an accredited institution.
- Licensure: Current state licensure (e.g. LCSW, LMSW) and meet all state required regulations to practice in a dialysis setting.
- Experience: Two years of clinical work experience, preferably in an ESRD setting.
- Certifications: Current CPR certification preferred.
