Key Responsibilities
  • Patient Assessment and Education: Conduct thorough assessments of patients' physical, emotional, and financial needs, and provide education on disease management and treatment options.
  • Care Planning and Coordination: Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans that address patients' unique needs and goals.
  • Communication and Interpersonal Skills: Demonstrate excellent communication and interpersonal skills to build strong relationships with patients, families, and healthcare professionals.

Requirements
  • Education: Master's degree in Social Work (MSW) from an accredited institution.

  • Experience: Two years of clinical work experience, preferably in an ESRD setting.
  • Certifications: Current CPR certification preferred.
