Job Summary

The Occupational Therapist - Post Acute Care Specialist provides direct rehabilitation care in occupational therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.

Key Responsibilities

  • Develop and implement individualized occupational therapy treatment plans for patients to improve their functional abilities and independence.
  • Conduct thorough assessments of patients' physical and cognitive abilities to identify areas for improvement.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other therapists, to ensure comprehensive care and effective communication.
  • Provide education and training to patients, families, and caregivers on occupational therapy techniques and strategies to promote independence and safety.
  • Document patient progress and outcomes in a timely and accurate manner, using standardized assessment tools and software.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures related to occupational therapy services.
  • Maintain current knowledge of best practices in occupational therapy and stay up-to-date with industry developments and research.

Requirements

  • NBCOT certification upon hire, with optional renewal.
  • Graduate of an accredited program in occupational therapy (BSOT or MSOT).
  • Current registration/licensure in the applicable state, with a commitment to maintaining an active license in good standing throughout employment.
  • One year of experience in post-acute care or a related setting, with a preference for candidates with a strong background in rehabilitation therapy.
  • CPR certification upon hire or during orientation, with a commitment to maintaining current certification throughout employment.
