Key Responsibilities- Work with patients, their families, and other healthcare professionals to provide comprehensive care during the final stages of life.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Provide education and support to patients and families on end-of-life care and related topics.
- Supervise and mentor licensed vocational/practical nurses and nursing aides as needed.
- Graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment.
- Valid state driver's license, automobile liability insurance, and dependable transportation.
- Minimum of one year of clinical experience, with home health or hospice experience preferred.
