Position Summary:

The Family Permanency Specialist is responsible for engaging with families to establish effective working relationships. Proficiency in computer skills is essential for completing necessary documentation. Experience with children with special needs is preferred, along with training in Reunification, Adoption, and Subsidized Guardianship.

Key Responsibilities:

  • Provide guidance on permanency matters; identify and address barriers to permanency, ensuring suitable placements for children; monitor and facilitate the completion of permanency processes for children awaiting reunification or adoption/subsidized guardianship.
  • Assist in connecting families with community resources to prepare for parental home readiness.
  • Support post-adoption and Guardianship connections with relevant child and family services prior to finalization.
  • Work to secure timely additional service resources to expedite permanency.
  • When reunification is not feasible, assist in listing children for adoption through appropriate registries.
  • Maintain accurate and confidential documentation, reports, and client records in compliance with relevant regulations and standards.
  • Adhere to accreditation standards.
  • Assist in completing required documentation and home studies to initiate the Adoption or Subsidized Guardianship subsidy process.
  • Submit and monitor subsidy packets pending approval from relevant child and family services.
  • Support completion of necessary documentation for out-of-state permanency options and collaborate with receiving states to eliminate barriers.
  • Facilitate connections between caseworkers, recruitment specialists, adoption staff, and community providers to expedite the permanency process.
  • This role does not involve case management.
  • Perform additional duties as assigned.

Qualifications:

Education and Experience:

  • Bachelor's degree in Social Work or a related human services field is required.

Skills and Competencies:

  • Basic proficiency in Microsoft Office Suite is required; familiarity with PowerPoint is a plus.
  • Excellent organizational and planning abilities.
  • Strong interpersonal and problem-solving skills.
  • Ability to maintain a client-centered focus.
  • Successful completion of background checks as required by relevant child and family services.
  • Valid driver's license and proof of vehicle insurance are mandatory.
  • CPR and First Aid certification is required.
  • Awareness and sensitivity to cultural and lifestyle differences among individuals.
  • Completion of FamilyCore training as required.

Physical Requirements:

  • Frequent sitting, lifting, and other physical activities typical of an office environment.
  • Occasional reaching and stooping.
  • Ability to operate standard office equipment.
  • Perform all job functions with or without reasonable accommodation.

Work Environment:

  • May encounter distractions and interruptions; potential for challenging client interactions.
  • Work occurs in various settings, including office and community environments.
