Key Responsibilities:

  • Coordinate, schedule, and assign palliative patients to healthcare providers.
  • Conduct follow-up communications with patients and families to gather feedback on the treatment plans.
  • Act as the primary contact, advocate, and informational resource for patients, their families, caregivers, payers, and community resources.
  • Manage referrals to appropriate agencies to help clients achieve their care plan goals.
  • Assist patients in navigating potential issues related to the healthcare system, including financial or social barriers.

Qualifications:

  • A minimum of two years of experience in Palliative, Hospice, or Home Care settings is required.
  • A Bachelor's degree in a health-related field is preferred.
  • Experience with medical records is preferred.
  • Familiarity with Aprima and HCHB is strongly preferred.
  • Demonstrated data analysis skills with prior use of electronic health records.
