Palliative Care Team Member
We're seeking a compassionate and detail-oriented Palliative Care Coordinator to join our team at AHN. As a key member of our care team, you will work closely with our field staff and leaders to provide exceptional support to our patients and their families.
In this role, you will be responsible for coordinating patient care, scheduling appointments, and assigning patients to providers. You will also conduct follow-up calls to patients and families to collect data and provide support.
Our ideal candidate will have a minimum of 2 years of experience in Palliative, Hospice, or Home Care, and a Bachelor's degree in a health-related field. Medical records experience and Aprima and HCHB experience are strongly preferred.
