Job Summary

As a Physician at HonorHealth, you will provide direct patient care in accordance with evidence-based treatment guidelines. You will deliver care in a team-centered approach, interacting with patients and families to keep them informed and engaged in their health care decisions.

You will provide counseling, education, and resource information on wellness, prevention of illness, and health maintenance. You will refer patients as appropriate for specialty care services and educational training for their conditions.

You will use disease registries to identify gaps and out-of-range values in care delivery, utilizing support services and care managers to improve metrics of healthcare status.

You will participate in improving perceived and actual access to care for patients through process improvement initiatives, delivering same-day appointments and extended hours of availability.

You will work with the process team to improve efficiencies of care delivery in the office setting, utilizing 'top of license' care by the office team.

You will maintain, monitor, and ensure accurate and timely documentation of patient encounter records to meet criteria set forth by coding guidelines, CMS, HIPAA, and other governmental regulations.

You will work with the Practice Development and Improvement Manager on the application process and assist the practice in achieving benchmarks toward Patient-Centered Medical Home status and NCQA certification.

You will participate in clinical integration, including process improvement, working with care managers, and coordinating transitions of care to specialists and inpatient services.

You will provide timely follow-up following inpatient discharge for transitions of care to the outpatient setting.

You will act in a fiscally responsible manner, striving to deliver value (quality/cost) to patients by giving the right care by the right individual at the right time.

Regularly attending meetings and events is essential for this role.
