Job Summary: We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents and their families.

Duties and Responsibilities:

  • Deliver exceptional care and services to residents, including assistance with activities of daily living, medication management, and transportation.
  • Develop and implement Individualized Service Plans (ISPs) that meet the unique needs and preferences of each resident.
  • Collaborate with community teams to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.

Requirements:

  • Dedication to serving seniors with excellent customer service skills.
  • A positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma/GED accepted and may be required per state regulations.
  • In states where appropriate, must maintain certifications.
  • Must be at least 18 years of age.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.

Benefits: We offer a competitive salary of $50,000 - $75,000 per year, depending on location, skills, and experience. Other benefits include medical, dental, vision, life, and disability plans, retirement savings plans, employee assistant program/discount program, paid time off, sick time, and holiday pay.
