About the Role

This is a full-time LPN position responsible for providing direct patient care, performing routine medical tasks, and collaborating with healthcare teams to ensure optimal resident outcomes. The successful candidate will have a strong passion for delivering compassionate care, excellent communication skills, and a commitment to maintaining a safe and clean environment.

Responsibilities
  • Patient Assessment and Care Planning: Participate in pre-admission assessments to identify potential residents' needs and develop personalized care plans.
  • Medication Administration: Administer medications according to physician orders, ensuring accuracy and adherence to the five rights of medication administration.
  • Health Promotion and Education: Provide educational programs to residents on healthy lifestyle choices, disease management, and wellness strategies.
  • Clinical Decision Making: Collaborate with healthcare teams to assess resident conditions, identify concerns, and develop interventions to address their needs.
  • Care Coordination: Coordinate with interdisciplinary teams to ensure seamless transitions of care, effective communication, and timely interventions.
  • Documentation and Reporting: Accurately document resident information, including assessment data, care plans, and progress notes.
Qualifications

To be considered for this role, you must possess:

  • A current license to practice as an LPN in the state of Michigan.
  • Graduation from an accredited School of Nursing.
  • Current CPR certification.
