Job Summary

We are seeking a highly skilled and compassionate Dietitian to join our team at Sodus Rehab & Nursing Center. As a Dietitian, you will play a vital role in ensuring the nutritional well-being of our residents, working closely with our healthcare team to provide personalized care.

Key Responsibilities
  • Assess and evaluate the nutritional status of residents, including laboratory data and fluid status.
  • Develop and implement individualized nutrition care plans, working closely with residents, families, and healthcare teams.
  • Provide education and support to residents and families on nutrition-related topics, including meal planning and self-management of nutrition care.
  • Collaborate with the Food Service Director to develop and implement therapeutic diets, ensuring the highest quality of care for our residents.
  • Participate in quality assurance and improvement initiatives, contributing to the development of policies and procedures that promote excellence in nutrition care.
  • Consult with healthcare providers to ensure seamless care coordination and communication.
Requirements
  • Bachelor's degree in Nutrition or Dietetics from a regionally accredited college or university.
  • Current certification as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) in New York State.
  • One to three years of direct care experience in long-term care, with a focus on nutrition and dietetics.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to interpret data and develop effective solutions.
