Nurse Practitioner-Medicine Specialist

Job Summary:The Nurse Practitioner-Medicine will provide comprehensive health care services to patients in a health care setting. This includes planning, coordinating, delivering, and managing health care services, as well as ensuring patient and family-centered quality care.
Responsibilities:- Provide continuous comprehensive medical and nursing care to patients and families.
- Manage health problems and coordinate health care for various patient populations.
- Develop and maintain partnerships with community-based organizations and make referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
- Evaluate and interpret patient data to plan and implement appropriate nursing interventions.
- Consult with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment.
- Formulate differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information.
- Develop, implement, and evaluate patient plan of care, determining effectiveness and modifying as per clinical standards and practice protocols, where needed.
- Order, conduct, and interpret diagnostic and laboratory tests, identifying and prescribing appropriate pharmacological and non-pharmacological interventions and therapies.
- Manage a patient session schedule, ensuring availability and continuity of care, and optimal availability and utilization of appropriate resources.
- Provide education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care.
- Provide education and counseling to patients and families regarding health status, treatment, and progress, medication management, disease prevention, and health promotion.
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
- Current certificate as a Nurse Practitioner issued by the NYSED.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
