ER and Critical Care Clinical Pharmacist

Join Trinity Health as a critical member of our interdisciplinary healthcare team in the Emergency Department and Critical Care units. As an ER and Critical Care Clinical Pharmacist, you will play a vital role in optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine.
Key Responsibilities:- Assess patient data to identify medication-related problems through profile reviews, pharmacy consults, and bedside interdisciplinary triage and rounding.
- Participate in medical emergencies and other formal alerts, such as trauma, stroke, sepsis, and STEMI.
- Review and interpret physician orders to ensure appropriateness for the patient's age, weight, and comorbidities.
- Check for drug incompatibilities and allergies, and take appropriate action when necessary.
- Consult with providers, nurses, and other healthcare professionals regarding pharmacotherapy optimization to achieve optimal patient outcomes.
- Provide accurate and timely responses to healthcare professionals and patients regarding drug information questions and facility medication use policies.
- Serve as a preceptor for pharmacy students and residents, and assist with orientation, instruction, and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists, and other health professionals.
- Assist nursing with medication administration when necessary, in accordance with regulatory and hospital policies.
- Document patient care activities, including therapy plans, monitoring, and follow-up, in the electronic medical record.
- Pharmacy degree (BS or Pharm.D.) from an ACPE-approved college of pharmacy.
- Licensed or eligible for pharmacist licensure in Iowa.
- Board of Pharmacy Specialties (BPS) certification required within 2 years of hire.
- BLS, ACLS, and PALS required within 6 months of hire.
- Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital pharmacy experience.
- Completion of PGY-2 Critical Care or Emergency Medicine Pharmacy Residency or 3 years of experience in Critical Care or Emergency Medicine is preferred.
