Clinical pharmacists play a vital role in optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine within interdisciplinary healthcare teams.
Key Responsibilities- Conduct daily medication therapy reviews, consults, and other clinical services for patients within designated areas of responsibility or targeted populations served.
- Participate in medical emergencies and other formal alerts as required.
- Review and interpret providers' orders to ensure appropriateness for patients' age, weight, and comorbidities.
- Consult with providers, nurses, and other healthcare professionals regarding pharmacotherapy optimization.
- Provide accurate and timely responses to healthcare professionals and patients regarding drug information questions and facility medication use policies.
- Serve as preceptor for pharmacy students/residents and assist with orientation, instruction, and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists, and other health professionals.
- Assist nursing with medication administration and document patient care activities in the electronic medical record.
- Responsible for the completion of all assigned work during the shift, maintaining the highest standards of quality and safety.
- Maintain current awareness and adhere to State and Federal laws, regulations, pharmacy policies, standard operating procedures, formulary, and pertinent clinical practice guidelines.
