We are seeking a highly skilled Clinical Pharmacist to join our team in Critical Care and Emergency Medicine. As a key member of our interdisciplinary healthcare team, you will be responsible for optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine throughout the ED and CC units.
Key Responsibilities- Assess patient data to identify medication-related problems through profile reviews, pharmacy consults, and bedside interdisciplinary triage and rounding.
- Participate in medical emergencies and other formal alerts such as trauma, stroke, sepsis, and STEMI.
- Review and interpret physicians' orders to ensure that the drug, dosage form, and dose are appropriate for the patient's age, weight, and comorbidities.
- Consult with providers, nurses, and other healthcare professionals regarding pharmacotherapy optimization to achieve optimal patient outcomes.
- Provide accurate and timely responses to healthcare professionals and patients regarding drug information questions and facility medication use policies.
- Serve as a preceptor for pharmacy students and residents, assisting with orientation, instruction, and training.
- Assist nursing with medication administration and document patient care activities in the electronic medical record.
- Responsible for completing all assigned work during the shift, maintaining quality and safety standards, and identifying process improvements.
- Pharmacy degree (BS or Pharm.D.) from an ACPE-approved college of pharmacy.
- Licensed or eligible for pharmacist licensure in Iowa.
- Board of Pharmacy Specialties (BPS) certification required within 2 years of hire.
- BLS, ACLS, and PALS required within 6 months of hire.
- Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital pharmacy experience.
