Job Summary

Mountain View Hospital is seeking a skilled Discharge Planner to join our team. As a key member of our healthcare team, you will work closely with physicians, healthcare personnel, and patients to ensure a smooth transition from acute care to home or community settings.

Key Responsibilities
  • Coordinate discharge plans and services with patients, families, and interdisciplinary teams to ensure a safe and effective transition.
  • Work with patients to maximize benefits from medical care, including reviewing and validating physician orders, reporting progress, and identifying areas for improvement.
  • Ensure cost-effective and appropriate healthcare services are provided to all patients, regardless of funding sources.
  • Collaborate with other departments to facilitate discharge planning and ensure seamless transitions.
  • Assess patients' physical, functional, social, psychological, environmental, and financial needs to identify areas for support and resources.
  • Act as a patient advocate, assisting with referrals to community resources and ensuring patients receive necessary medical, nursing, or other healthcare services upon discharge.
  • Maintain accurate patient records, ensuring confidentiality and compliance with regulatory requirements.
  • Contribute to case management activities, assessing, planning, implementing, coordinating, monitoring, and evaluating patient needs for services during and after hospitalization.

Qualifications
  • Graduate from an accredited school of nursing with current licensure as an LPN or Registered Nurse in the state of Idaho.
  • Preferred experience in Case Management, with strong knowledge of modern nursing principles, techniques, and procedures for patient care.
  • Basic computer knowledge and effective written and oral communication skills.
