Geriatric Nurse Specialist

Unit: 6A - Acute Care for the Elderly (ACE) Unit
We are seeking a compassionate and skilled Registered Nurse (RN) to join our interdisciplinary team in providing high-quality care to older adults facing acute illnesses. As a member of our 6A team, you will work closely with patients, families, and healthcare professionals to deliver patient-centered care that supports functional independence, education, and safety.
Responsibilities:
- Manage a caseload of 5 medical patients per shift, working collaboratively with an interdisciplinary team to develop and implement individualized care plans.
- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient outcomes.
- Engage in relationship-based care, building strong relationships with patients, families, and peers, guided by our values of love and excellence.
- Participate in unit-based quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Delaware RN License or Compact Nursing License.
- BSN required; ADN candidates must obtain a BSN within 3 years of hire date.
- One year of experience as an RN; gerontology certification preferred.
- BLS required.
