Primary Care Physician

Key Responsibilities:- Provide comprehensive care to patients with acute and chronic illnesses, focusing on geriatric medicine and adult health.
- Develop and implement personalized care plans, working closely with patients, families, and care teams.
- Lead by example, demonstrating exceptional clinical skills, empathy, and a patient-centered approach.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
- Stay up-to-date with the latest evidence-based medicine, clinical guidelines, and best practices in geriatric care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Current, active MD licensure in the state of employment.
- A minimum of 1 year of clinical experience in geriatric, adult, or family practice settings.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Basic Life Support (BLS) certification and a current DEA number for schedule II-V controlled substances.
