Job SummaryWe are seeking a highly skilled and compassionate Primary Care Physician to join our team at ChenMed. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our seniors, working collaboratively with our care teams to achieve exceptional health outcomes.
Key Responsibilities- Provide comprehensive primary care to patients with acute and chronic illnesses, including geriatric, adult, and family practice settings.
- Develop and implement individualized care plans, working closely with patients, families, and care teams to achieve optimal health outcomes.
- Lead and manage care teams, including care promoters, care facilitators, and care coordinators, to ensure seamless patient care.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.
- Collaborate with hospitalists, specialists, and other healthcare professionals to ensure comprehensive patient care.
- Stay up-to-date with the latest clinical, nutritional, and lifestyle-based interventions to improve patient outcomes.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
- Current, active MD licensure in the State of employment required.
- A minimum of 1-year clinical experience in geriatric, adult, or family practice setting preferred.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred, Board Eligibility required.
- Basic Life Support (BLS) certification required.
- Must have a current DEA number for schedule II-V controlled substances.
