Clinical Pharmacotherapy Specialist, Adult Transplant Expert

Job Summary:
We are seeking a highly skilled Clinical Pharmacotherapy Specialist to join our team at NYU Langone Health. As a key member of our healthcare team, you will be responsible for developing and maintaining a high-level practice in the area of Adult Transplant Services.
Key Responsibilities:
- Develop and Implement Clinical Practice Guidelines: Create and implement clinical practice guidelines that enhance pharmaceutical care and medication safety for adult transplant patients.
- Pharmacy Resident Precepting and Student Mentoring: Provide guidance and mentorship to pharmacy residents and students, ensuring they receive comprehensive training and support.
- Medication Use Evaluations and Clinical Interventions: Conduct medication use evaluations and provide clinical interventions to optimize patient care and medication safety.
- Quality Assurance and Quality Improvement: Participate in quality assurance and quality improvement initiatives to enhance the quality of care and medication safety.
- Collaboration and Communication: Work collaboratively with healthcare teams to ensure seamless communication and coordination of care.
Requirements:
- Doctor of Pharmacy Degree: Must have a Doctor of Pharmacy degree from an ACPE-accredited school of pharmacy.
- New York State Pharmacist Licensure: Must be licensed to practice pharmacy in the state of New York.
- PGY1 and PGY2 Pharmacy Residency Training: Must have completed PGY1 and PGY2 pharmacy residency training or equivalent salaried professorship at a school of pharmacy.
- Board Certification in Pharmacotherapy: Must be board certified in pharmacotherapy or other specialty within 18 months of hire.
