**Job Summary: Master Care Coordinator**

We are seeking a highly skilled and compassionate Master Care Coordinator to join our team at Springpoint Living in Denville, NJ. This part-time role will play a vital part in the Social Services department, focusing on providing medically related social services to promote the physical, mental, and psychosocial well-being of our residents.

**Responsibilities:**

* Assess the social and emotional needs of each resident in a timely and efficient manner.
* Conduct thorough documentation of resident interactions and interventions.
* Engage in confidential and private interviews with residents and their families as necessary.
* Collaborate with residents and their families to develop personalized social services programs.
* Refer residents and their families to relevant social service programs and agencies when appropriate.
* Maintain strong, professional relationships with medical representatives and other healthcare organizations.
* Participate in community planning initiatives that align with the interests of the facility and the needs of our residents and their families.
* Work closely with consultants to implement recommended changes and ensure seamless integration with the Director of Social Services.
* Ensure the emotional, educational, social, and spiritual needs of our residents are met.
* Support the Director of Social Services and Admissions Coordinator during the adjustment period for new residents.
* Collaborate with the Resident Review Team to guarantee that each resident's needs are met.

**Education and Experience:**

* Bachelor's or Master's degree in Social Work required.
* Must be a Certified Social Worker (CSW), Licensed Social Worker (LSW), or Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
* Experience in Long-Term Care settings is highly preferred.

**Salary Range:** $2,240 - $2,496 per week (approx. $28-$31 per hour).
