Job Summary

We are seeking a skilled Intake Assessment Specialist to join our team. The successful candidate will be responsible for facilitating the intake process, conducting initial evaluations, and providing information to physicians for Level of Care (LOC) determination.

Key Responsibilities

  • Facilitate response to requests for services, including initial response, assessment, and referral process
  • Conduct initial evaluation, initiate assessment, document patient symptomology, functioning, and needs
  • Provide information to MD for LOC determination
  • Demonstrate excellent customer service skills in assisting patients, physicians, visitors, and coworkers
  • Maintain all preadmission and admission paperwork with accuracy

Requirements

  • Degree in social work or counseling required
  • Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred
  • Current unencumbered clinical social work or counseling license
  • CPR and Handle with Care (HWC) certified within 30 days
