Registered Nurse Lead-Inpatient Border Full Time Nights

As a Registered Nurse Lead, you will be responsible for coordinating patient care in the Inpatient Border Unit. This includes performing comprehensive assessments, communicating changes in patient condition, and utilizing evidence-based care to derive realistic outcomes and document them as measurable goals on the plan of care.
You will also identify and prioritize patient needs and/or nursing diagnosis for patients and document them. Collaboration with patient, family, and other members of the health care team is essential when planning patient care and documenting accordingly.
Assessing patients with regard to discharge planning and making the appropriate referrals as necessary is also a key responsibility.
Additionally, you will evaluate and document the patient response to nursing interventions and alter the plan of care as necessary.
Ensuring a completed discharge instructions, medication reconciliation, and providing information to patient/family regarding the use of the patient portal and other pertinent resources is also a critical aspect of this role.
