Responsibilities:

  • Relationship to Patient/Family and Community:
    • Perform comprehensive assessments on assigned patients, documenting findings and communicating changes in patient condition to the healthcare team.
    • Collaborate with patients, families, and other healthcare professionals to develop and implement individualized care plans.
    • Identify and prioritize patient needs, documenting nursing diagnoses and developing realistic goals for patient care.
  • Care Coordination:
    • Utilize evidence-based practice to develop and implement nursing interventions, modifying care plans as necessary to achieve patient goals.
    • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
    • Assess patients for discharge planning, making referrals as necessary to ensure a smooth transition to post-acute care.
  • Patient Education/Ethic and Diversity:
    • Support patient and family education, identifying learning needs and developing individualized education plans.
    • Utilize teach-back methodology to advance patient education on medication side effects and new medication education.
    • Respect and value patient diversity, incorporating cultural vision into care planning and delivery.
  • EBP and Research:
    • Implement nursing interventions consistent with established care plans, utilizing evidence-based practice to inform decision-making.
    • Maintain and advance professional nursing practice through ongoing education and participation in clinical research.
  • Healing Safe Environment:
    • Demonstrate safe medication administration through the use of bar coding.
    • Protect patients by maintaining a safe environment of excellence.
    • Participate in unit-based and organizational quality initiatives to drive exceptional care.
  • Relationship to Colleagues:
    • Collaborate with healthcare team members to achieve shared goals and objectives.
    • Provide leadership and direction to patient care technicians, maintaining overall accountability for patient care.
    • Participate in peer evaluation and feedback, contributing to a culture of excellence and continuous improvement.
  • Resource Utilization:
    • Ensure accurate documentation of care orders in the electronic medical record.
    • Participate in value-based initiatives to optimize resource utilization and reduce costs.
  • Use of Technology:
    • Utilize electronic documentation and other technologies to enhance patient care and streamline workflows.
    • Participate in the advancement and optimization of the medical record through CEOC activities.
  • Relationship to Self:

Qualifications:

  • Education: Bachelor of Science in Nursing (BSN)
  • License/Certification: Current State of Illinois Registered Professional Nurse, Basic Life Support
  • Skills: Strong interpersonal skills, ability to collaborate with others, strong analytic abilities

Preferred Qualifications:

  • Certification: National Certification in area of specialty
