Team Case Manager

Richmond Residential Services (RRS) is seeking a dedicated Team Case Manager to join our team. This role will provide case management support for adults with intellectual disabilities living in community-based settings. As a Team Case Manager, you will be responsible for managing a caseload of 7-10 individuals, completing quarterly reports, plan updates, and fall risk plans/assessments.
Key Responsibilities:
- Manage a caseload of 7-10 individuals, ensuring timely completion of quarterly reports, plan updates, and fall risk plans/assessments.
- Maintain electronic health care records for assigned individuals, uploading data into the EHR system as needed.
- Provide support at annual meetings, client care specific meetings, and participate in root cause analysis for individuals on assigned caseloads.
- Assist with direct care support to individuals at assigned programs, including medical appointments, community activities, and DSP coverage.
- Participate in On-Call responsibilities for assigned RRS Director, Team, and respond to individual care needs and critical staffing issues as needed.
- Oversight of assigned program health appointment and records system.
Requirements:
- Bachelor's Degree in Human Services preferred, or a field related to community services.
- One or more years of directly related experience or any combination of training and experience providing required knowledge, skills, and abilities.
- Valid Virginia Driver's license and acceptable Department of Motor Vehicles Driving record and dependable transportation.
- Able to successfully pass a background check and other pre-employment screenings.
Estimated Salary Range:$21.00 per hour, depending on experience, plus $1.50 shift differential for hours worked from 4P to 7A.
