Position Overview
The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. The LPN is dedicated to delivering patient care that addresses physical, emotional, developmental, psychosocial, and educational needs across various age groups. This role is pivotal in ensuring the coordination and provision of exceptional care. Responsibilities include preparing patients for examinations, assisting during medical procedures, coordinating diagnostic tests and follow-up care, and documenting treatments in medical records. The LPN also plays a key role in educating patients and their families. Participation in staff meetings, training sessions, special projects, and ongoing education is expected. Additionally, the LPN may act as a mentor for newly hired colleagues.
Key Responsibilities
DIRECT PATIENT CARE
- Evaluates and assists in managing the comprehensive needs of patients, following physician directives with RN oversight and employing the nursing process.
- Executes routine nursing tasks to fulfill patient requirements under RN supervision.
- Adheres to established productivity benchmarks.
- Demonstrates proficiency in accessing and de-accessing ports, including flushing lines and maintaining sterile techniques.
- Exhibits knowledge and skill in medication administration as per the defined scope for the organization.
- Seeks guidance, training, or direction from leadership as necessary.
- May assign tasks to unlicensed personnel, as appropriate and within their practice scope.
- Provides patient-centered care that is sensitive to population-specific characteristics (e.g., age, gender, medical conditions).
- Collaborates with patients, significant others, and/or family members to create a comfortable and safe care environment, focusing on the patient's goals in the care plan.
- Coordinates care and services with the RN and other healthcare team members.
- Communicates changes in patient conditions to the RN, physician, midlevel provider, or clinical supervisor as appropriate.
- Works with the RN to develop and adjust individualized care plans based on patient needs and physician orders.
- Acts as an educator for patients to promote self-care and support for caregivers.
- Utilizes teach-back methods to ensure understanding of self-care among patients and/or caregivers.
- Participates in the orientation and training of new staff and nursing students as appropriate.
- Responsible for attending continuing education and in-service training as required.
- Commits to staying updated on clinical knowledge relevant to patient care.
- Ensures accurate and timely completion of required documentation.
- Demonstrates understanding and application of regulatory guidelines.
- Responsible for checking communications and returning patient calls as necessary.
- Checks work-related emails daily.
Practical Nursing Diploma Required
Licenses and Certifications Required
All certifications must be obtained upon hire unless otherwise specified.
- Licensed Practical Nurse
- Basic Life Support (BLS) Certification - Instructor
Previous experience in healthcare is preferred, with a focus on oncology, ambulatory, or outpatient settings.
Essential Skills
Strong leadership qualities, problem-solving abilities, excellent organizational and time management skills, along with proficient assessment and clinical skills. Effective written and verbal communication skills are also essential.
