Travel Nurse Source is working with Cynet Health to find a qualified Home Health RN in a dynamic and rewarding environment. As a Home Health RN, you will administer skilled nursing care to patients on an intermittent basis in their place of residence, under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.
Responsibilities:- Complete thorough nursing assessments on admission and re-evaluate patient nursing needs on succeeding visits.
- Initiate and update the plan of care for SOC (Start of Care), ROC (Resumption of Care), RECERT (Recertification of Care), and DC (Discharge) using Medicare OASIS Forms.
- Provide services or treatments requiring substantial and specialized nursing skill according to the written patient care plan and signed plan of treatment.
- Case manage all therapies, including LPN, PT, OT, ST, and CNA.
- Prepare clinical and progress notes for each patient visit and summaries of care conferences on assigned patients in a timely manner.
- Graduate of an accredited school of nursing.
- License in good standing.
- One year of recent experience in Home Health.
- AHA BLS required.
