We are looking for a highly motivated and detail-oriented Service Coordination and Case Management Professional to join our team at Belvedere Home Care. As a member of our team, you will be responsible for coordinating services for individuals with disabilities, including reviewing and updating Service Plans, conducting regular meetings with participants, and ensuring that all necessary documentation is completed accurately and efficiently.

Successful candidates will possess excellent communication and organizational skills, as well as the ability to work effectively in a fast-paced environment. A minimum of one year of experience in case management/service coordination is required, although additional education or experience may be necessary depending on the individual's qualifications.

This full-time position offers a salary range of $18.00 to $20.00 per hour, as well as a comprehensive benefits package that includes medical, dental, vision, and life insurance, as well as paid time off.
