Job Description

We are seeking a highly motivated and detail-oriented Service Coordinator to join our team. As a Service Coordinator, you will be responsible for delivering high-quality service coordination duties and maintaining regulatory compliance for individuals living in the community with traumatic brain injuries or enrolled in the Nursing Home Transition and Diversion waiver.

Responsibilities

  • Establishing and maintaining regular contact with individuals we support and advocating on their behalf
  • Developing and updating services plans with strict deadlines
  • Linking individuals and applying for needed services
  • Assisting with maintaining benefits and collaborating with other services providers

Requirements

">

To be successful in this role, you must have:

">
  • An Associate's Degree required, Bachelor's Degree preferred
  • At least 2 years of experience providing case management/service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors
  • Ability to handle multiple tasks simultaneously, effectively communicate, make decisions and problem solve
  • Must be familiar with Microsoft Word and Excel
  • Reliable transportation to travel to locations throughout the greater Capital Region area

Salary and Benefits

The salary for this position is $21.98-$24.67 per hour, commensurate with experience. We also offer a range of benefits, including health insurance, paid time off, and opportunities for professional growth and development.
