Job Summary

We are seeking a highly skilled MDS Coordinator to join our team at Keizer Campus Operations LLC. As an MDS Coordinator, you will play a critical role in developing plans of patient care for each resident and reviewing their medical and nursing treatments to ensure they align with their care plan and wishes.

Key Responsibilities
  • Develop and Implement Care Plans: Collaborate with healthcare teams to create and implement individualized care plans that meet the unique needs of each resident.
  • Review and Revise MDS: Conduct thorough reviews of the Minimum Data Set (MDS) to ensure accuracy and completeness, and make revisions as necessary.
  • Lead Nursing Operations: Supervise and support nursing staff to ensure high-quality patient care, and make recommendations for process improvements.
  • Communicate Effectively: Develop and maintain strong relationships with residents, families, and healthcare teams to ensure seamless communication and care coordination.
  • Stay Up-to-Date with Industry Developments: Participate in ongoing education and training to stay current with changes in healthcare laws, regulations, and best practices.
Requirements
  • Nursing Degree: Bachelor's degree in nursing from an accredited institution.
  • Current RN License: Active, unencumbered license to practice as a registered nurse in this state.
  • Leadership Experience: At least one year of experience in a supervisory or leadership role in a healthcare setting.
  • Rehabilitative and Restorative Nursing Knowledge: Training in rehabilitative and restorative nursing practices, with a focus on promoting resident independence and well-being.
  • Electronic Medical Records Experience: Proficiency in electronic medical records and computer documentation systems.
