RN MDS Coordinator Job Summary

We are seeking a skilled and compassionate RN MDS Coordinator to join our team at Avamere Living. As a key member of our healthcare team, you will play a vital role in developing and implementing patient care plans that meet the unique needs of our residents.

Key Responsibilities:
  • Participate in developing and reviewing patient care plans to ensure they align with resident goals and preferences.
  • Complete and implement MDS assessments, including CAAs and triggers, to ensure accurate and comprehensive care planning.
  • Review and analyze nursing notes to ensure they are informative and descriptive of care provided and resident response.
  • Develop and maintain positive working relationships with all team members to ensure resident needs are met.
  • Make recommendations for new or updated procedures, policies, and practices to ensure high-quality patient care.
  • Participate in discharge planning reviews and prepare reports for the Care Plan Committee as directed.
  • Attend continuing education programs to stay current with industry developments and participate in in-service training for nursing personnel.
  • Delegate, train, evaluate, and support RN, LPN, and CNA personnel.
  • Provide direct nursing care as needed and assist the Director of Nursing Services as required.
  • Maintain confidentiality of resident care information in accordance with HIPAA guidelines.
Requirements:
  • Nursing degree from an accredited college or university.
  • Current, unencumbered, active license to practice as an RN.
  • At least 2 years of experience as a supervisor in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Knowledge of nursing and medical practices, laws, regulations, and guidelines.
  • Experience with electronic medical records and computer documentation systems.
  • Passion for caregiving and serving our senior resident community.
