Job Summary: We are seeking a highly skilled Medical Social Worker to join our team. In this critical role, you will work closely with our healthcare team to develop treatment plans and determine services needed by patients and families. Utilizing your excellent communication and interpersonal skills, you will gather clinically relevant information and provide psychosocial assessments to support informed decision-making. Your primary focus will be on achieving utilization and quality goals through effective planning, prioritizing, and organizational strategies.

Key Responsibilities: As a Medical Social Worker, you will consult with attending physicians to coordinate discharge plans for patients, identify opportunities for improvement, and participate in the development and implementation of action plans to make improvements. You will also contribute to patient outcome measurement, participate in Discharge Planning Team meetings, and perform other duties as assigned by authorized personnel or required in an emergency.

Our Benefits: We offer a competitive salary range of $70,000 to $90,000 annually, depending on experience.
