Care Management Specialist RN Job Summary

We are seeking a highly skilled Care Management Specialist RN to join our team at Centene Corporation. As a Care Management Specialist RN, you will play a critical role in assessing and coordinating medical and supporting services for complex populations.

About You: To be successful in this role, you will need a graduate degree in Nursing or related field and 4-6 years of experience in care management. You must also possess RN licensure or NP certification.

Key Responsibilities:

  1. Evaluate service needs and develop plans for optimal outcomes
  2. Assess ongoing long-term care plans and make recommendations for improvement
  3. Collaborate with members, caregivers, and care providers to ensure adequate care
  4. Monitor member status and revise care plans as necessary
  5. Review member data to identify trends and improve operating performance
