Key Responsibilities:
  • Collaborate with healthcare providers to identify and support patients' needs.
  • Develop and implement effective discharge plans to ensure a safe transition of care.
  • Identify and connect patients with community resources to achieve desired outcomes.
  • Communicate with payers and patients regarding coverage determinations.
  • Participate in multidisciplinary rounds to share information and facilitate the healthcare team.
Requirements:
  • At least one year of experience in an acute medical hospital setting or three years in a sub-acute setting.
  • Graduate from an accredited school of nursing or Bachelor in a related behavioral/social science field.
  • Valid AZ RN license and BLS certification.
  • ACMA or CCM certification desired.
