Job Description:
We are seeking an experienced Housing Support Specialist to join our multidisciplinary team. The successful candidate will be responsible for providing rehabilitation and support services to consumers, with a focus on housing and independent living. This includes conducting assessments, developing treatment plans, and providing education and support to consumers and their families.

Responsibilities:
- Provide service coordination and case management for assigned groups of consumers
- Develop, write, implement, evaluate, and revise overall treatment goals and plans in collaboration with the consumer and the ITT
- Educate and support consumers' families, and advocate for consumers' rights and preferences
- Assist in conducting comprehensive assessments and provide ongoing assessment of consumers' psychiatric symptoms and response to treatment

Qualifications:
- Bachelor's degree in Human Services/Social Services, Healthcare, or a related field
- Experience working in the Social Services/Mental Health/Substance Abuse field
- Valid Driver's License required
