We are seeking a compassionate and dedicated Community Support Specialist to join our team at Asociación Puertorriqueños en Marcha (APM) CUA. As a Community Support Specialist, you will play a vital role in supporting the safety, stability, and well-being of children and families in the North Philadelphia community.

About the Role

This position requires a minimum of 2 years' experience in human services, preferably with a focus on child welfare, mental health, or social work. You will be responsible for developing and implementing service plans for clients, providing individual or family counseling, and making referrals to various support agencies.

Responsibilities
  • Conduct thorough assessments of clients' needs and develop personalized service plans.
  • Provide individual or family counseling, as needed, to address emotional, behavioral, or psychological issues.
  • Make referrals to community resources, such as food banks, housing assistance programs, or healthcare services.
  • Collaborate with other professionals, including caseworkers, therapists, and medical professionals, to ensure comprehensive support for clients.
  • Document client progress, maintain accurate records, and report on program outcomes.

You will also have opportunities to participate in case conferences, school conferences, and court appearances, as needed, to advocate for the best interests of clients.

Requirements
  • Bachelor's degree in Social Work or a related field.
  • At least 2 years' experience in human services, preferably with a focus on child welfare, mental health, or social work.
  • Valid driver's license with a clean driving record.
