Job Overview

Position Summary

The Speech Language Pathologist (SLP) plays a crucial role in delivering home health services within community and facility environments. This position is essential for providing therapeutic interventions to patients enrolled in home health and hospice programs, following the directives of the attending physician.

Key Responsibilities

  1. Conduct thorough assessments of speech and language capabilities, evaluating the patient's rehabilitation potential, home safety, and psychosocial needs.
  2. Analyze evaluation results to formulate a personalized care plan with specific therapy objectives aimed at enhancing communication and swallowing functions.
  3. Implement prescribed treatment strategies, which may involve therapeutic exercises, adaptive equipment recommendations, and instructing patients and families on home care practices.
  4. Regularly reassess patient progress and modify the therapy plan as necessary, in collaboration with the physician.
  5. Facilitate coordination of speech therapy services with nursing, social work, and other healthcare professionals to ensure comprehensive care delivery.
  6. Engage in interdisciplinary team meetings to discuss patient care and treatment plans.
  7. Document patient responses to treatment and any significant changes in their condition, while maintaining accurate clinical records.
  8. Provide guidance and training to other healthcare team members on speech therapy techniques.
  9. Maintain current Basic Life Support (BLS) certification.
  10. Perform additional duties as required.

Qualifications

Required:

  • Master's Degree in Speech Language Pathology.
  • Active Speech Language Pathology license in Washington State.
  • Certification of clinical competence from the American Speech and Hearing Association.
  • Proficient communication skills in English, including listening, reading, speaking, and writing.
  • Basic computer skills, including proficiency in Windows and electronic communication.
  • Valid driver's license and access to a vehicle with required insurance coverage.

Desired:

  • At least one year of recent experience in speech language pathology.
  • Experience in home care, acute care, or skilled nursing settings.
