Job Description

Overview

The Licensed Speech Language Pathologist delivers home-based therapy services in both community and facility environments. This role involves providing therapeutic interventions to patients enrolled in home health and hospice programs, under the guidance of the patient's physician. The responsibilities include diagnosing communication and swallowing disorders and implementing therapeutic strategies for patients referred by a Licensed Health Care Professional (LHCP). Quality care is ensured in compliance with state, federal, and accreditation regulations, as well as the mission of Confluence Health.

Position Reports To: Rehabilitation Supervisor and agency leadership


Key Responsibilities

  1. Conducts thorough assessments of speech and language capabilities, evaluates rehabilitation potential, home safety, equipment requirements, psychosocial conditions, and the need for community resources and home health/hospice services through skilled observation and age-appropriate testing.
  2. Analyzes evaluation and testing outcomes to create a tailored plan of care with specific therapy objectives aimed at helping patients achieve, regain, or maintain optimal function and safety within their home and community.
  3. Implements skilled treatment interventions as prescribed by the physician, which may include therapeutic exercises, modalities, recommendations for adaptive equipment, compensatory techniques, and instructing patients and families on home programs to continue in the therapist's absence, when necessary.
  4. Regularly reassesses the patient's functional abilities and the effectiveness of the speech therapy plan, making necessary adjustments with the physician's consent.
  5. Coordinates the provision of speech therapy services with nursing, social work, aides, therapy, equipment suppliers, community resources, and transitions of care to ensure effective resource utilization and timely achievement of rehabilitation and/or palliative care goals.
  6. Participates in interdisciplinary team meetings and agency meetings as required.
  7. Documents and reports to the physician regarding the patient's response to treatment, any changes in condition, suggested modifications to the patient's orders, and the implementation of discharge planning procedures.
  8. Maintains comprehensive documentation of evaluations, reassessments, subsequent visits, care coordination, discharge or transfer summaries, and other clinical records in the electronic medical record system.
  9. Provides instruction and supervision to other health team members in speech therapy techniques performed as part of the overall speech therapy plan of care.
  10. Maintains active Basic Life Support (BLS) certification.
  11. Performs additional duties as assigned.

Qualifications

Required:

  • Master's Degree in Speech Language Pathology
  • Current Washington State Speech Language Pathology license.
  • Certification of clinical competence from the American Speech and Hearing Association
  • Effective communication skills: Ability to listen, read, speak, and write proficiently in English. Adequate hearing and visual acuity to detect changes in patient conditions and perform assessments. Strong interpersonal skills and the ability to collaborate with health team members.
  • Basic computer skills related to Windows and app navigation, including mouse and screen usage, keyboarding, email communication, and password management.
  • Valid driver's license and access to an insured vehicle meeting minimum legal coverage requirements.

Desired:

  • One year of recent experience in speech language pathology.
  • Prior experience in home care, acute care, or skilled nursing settings.
