Job Summary

We are seeking a skilled Speech Language Pathologist to join our team at Confluence Health. As a Speech Language Pathologist, you will provide home care services to patients in community-based and facility settings, working under the direction of a patient's physician. Your services will include diagnosing disorders and providing therapeutic interventions to patients with communication and/or swallowing disorders, as referred by a Licensed Health Care Professional.

Key Responsibilities:

  1. Conduct comprehensive speech and language patient assessments, evaluating rehab potential, home safety, equipment needs, psychosocial status, and community resource needs.
  2. Develop and implement individualized plans of care with patient-specific therapy goals to help patients develop, regain, or maintain optimal function and safety in their home environment and community.
  3. Administer skilled treatment interventions, including therapeutic exercises, modalities, adaptive equipment recommendations, and compensatory techniques, as prescribed by the physician.
  4. Reassess patient status and effectiveness of the speech therapy plan of care, making revisions with physician approval as needed.
  5. Coordinate delivery of speech therapy services with nursing, social work, aide, therapy, equipment suppliers, community resources, and transitions of care to ensure effective resource utilization and timely attainment of rehabilitation and/or palliative care goals.
  6. Participate in weekly interdisciplinary team meetings and agency meetings as assigned.
  7. Record and report patient reactions to treatment, changes in condition, suggested changes in orders, and implementation of discharge planning procedures.
  8. Maintain accurate documentation of evaluation, reassessment, subsequent visits, coordination of care, discharge or transfer summaries, and other clinical record documentation in the electronic medical record.
  9. Supervise and instruct other health team members in speech therapy procedures and techniques as an adjunct to the regular speech therapy plan of care.
  10. Required to maintain active Basic Life Support (BLS) certification.
  11. Other duties as assigned.

Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.

Qualifications
  • Masters' Degree in Speech Language Pathology
  • Current WA State Speech Language Pathology license
  • Certification of clinical competence by the American Speech and Hearing Association
  • Effective communication and interpersonal skills, with ability to work collaboratively with health team members
  • Basic computer/electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication, and password management
  • Current, valid driver's license and access to an insured vehicle with minimum coverage by law

Desired qualifications include one year of recent speech language pathology experience and previous home care, acute care, or skilled nursing experience.
