Job Summary
We are seeking a highly skilled Public Health Nurse to join our team in Dublin, GA. As a Public Health Nurse, you will provide direct nursing care to clients enrolled in the Ryan White program, including medical and non-medical case management, treatment adherence, evaluation, counseling, and linking clients to sub-specialty services.
Job Responsibilities
This is a dynamic role that requires a strong nursing background, excellent communication skills, and the ability to work in a fast-paced environment. Key responsibilities include:
- Providing direct nursing care, including physical assessments, education, treatment, and developing and documenting a plan of care consistent with the client's level of need.
- Screening and referring clients to other community service providers as needed, and assisting with referrals and follow-up treatment planning as indicated.
- Providing sexual health promotion and prevention education, including counseling clients and family regarding the importance of medication adherence for viral suppression.
- Participating in staff meetings and assisting with developing and implementing quality improvement projects as needed.
- Maintaining accurate medical records, documenting as a must, and assisting with data entry and quality assurance monitoring.
Required Skills and Qualifications
To be successful in this role, you must have:
- An Associate degree in nursing from an accredited college or university
- One (1) year of nursing experience and completion of a PHN course within one year of hire
- A current Georgia Registered Professional Nurse license and CPR certification
