Hospice of the Western Reserve is a leading organization in the field of hospice care, dedicated to providing comfort and support to individuals and families affected by terminal illnesses. We are seeking an experienced Social Worker to join our dynamic team, providing counseling services and medical social work to terminally ill persons and families in various settings across Lorain County, Cuyahoga County (West), Summit, and Medina County.

About the Role
In this role, you will work collaboratively with our transdisciplinary team to provide emotional, spiritual, and psychosocial support to patients and their families. You will assist with group counseling and support programs, maintain accurate records, participate in care conferences and bereavement programs, and support patients and family members with referrals to community resources. Additionally, you will have the opportunity to work in a variety of settings, including office-type settings and patient care environments.

Requirements
  • Must be licensed in the State of Ohio
  • Hold a Master's degree in Social Work or related field
  • Possess 2+ years of experience in counseling or related work with individuals, families, and groups
  • Have knowledge of community resources and ability to connect patients with necessary support services
