Ready offers two clinical service programs: the Home-Based Recovery program and the Rapid Advanced Care program. When combined, these programs create a unique solution for reducing healthcare utilization and preventing readmissions in your patient population.

Job Summary:

We are seeking a skilled and compassionate Home-Based Care Provider to join our dynamic team of dedicated Physician Assistants and/or Nurse Practitioners. As a Home-Based Care Provider, you will travel to patients' homes or wherever they are to deliver exceptional care and coordinate patient care across multiple providers and settings to achieve integrated healthcare and enhance patient outcomes.

Key Responsibilities:

  • Conducting comprehensive health assessments and diagnostic tests to diagnose and treat acute symptoms
  • Providing therapeutic interventions such as IV fluids, medications, wound care, and arranging follow-up care
  • Incorporating patient goals and palliative care principles into the care plan
  • Educating patients and families on managing acute illnesses and overall well-being
  • Collaborating with the care team and communicating effectively with all involved in patient care

Requirements:

  • Current state licensure as a Physician Assistant or Nurse Practitioner
  • Minimum of two years of relevant clinical experience
  • Current BLS certification, ACLS preferred
  • Prescriptive authority and DEA registration
  • Valid driver's license with a clean record
  • Ability to work in nonclinical environments and patients' homes
