These programs provide a unique utilization reduction and readmission prevention solution for high-risk populations.
Our Approach:
We believe in treating patients in the comfort of their own homes, avoiding the conventional acute care journey from ER to admission to post-acute care. Our team of compassionate advanced practitioners works in partnership with field responders to provide integrated medical services, ensuring patients receive the highest quality care and achieve improved outcomes.
Key Responsibilities:
- Working as part of a clinical care team with a field responder to deliver Rapid Advanced Care and Recovery at Home in patients' homes by:
- Performing problem-focused assessments and various health screenings to ensure accurate diagnoses and treatments
- Ordering, performing, and analyzing test results to support medical decision-making
- Providing therapeutic interventions, such as IV fluids and medications, splinting, suturing, acute wound care, and arranging for follow-up care
- Organizing mobile testing to ensure the most appropriate therapeutic interventions
- Integrating patients' goals of care into the care plan, applying palliative principles when necessary
- Using critical thinking skills and following evidence-based standards of practice
- Educating patients and families on how to continue treatment and manage their health and well-being
- Communicating effectively with patients, families, healthcare agents, primary care providers, and other individuals involved in patient care
- Real-time communication with referring clinicians and closing the loop with referrers once the patient has been evaluated and a plan initiated
- Effectively communicating with patients, caregivers, and loved ones in diverse cultural, socioeconomic, and age groups
- Attending and participating in clinical conferences, training, and huddles
- Identifying potential technical or patient care issues and working with leadership to make recommendations for patient safety
- Supporting interprofessional teams and leadership to evaluate and measure the impact of new approaches
- Supporting clinical guideline development, clinical pathways, disease management, transitional care management, and drug use programs

- Competent in skills including, but not limited to, IV placement, phlebotomy, I&D, suturing, suctioning, foley catheter care, GT care, trach care, and central line care
Key Performance Indicators (KPIs):
- Quality outcomes such as readmission rate, NPS, complication rate, and return rate
- Attendance and participation in monthly clinical department meetings
- Participation in Quality Improvement evaluation, improvement, and remediation when required
- Customer Service Satisfaction as measured by individual items such as NPS scores, surveys, written or verbal compliments or complaints
- Timely completion of medical records
Requirements:
- Current State Licensure as a Physician Assistant or Nurse Practitioner
- Minimum of two years of experience as a PA or NP in the Emergency Department, Urgent Care, Hospitalist, Cardiac, Pulmonary, or other relevant areas
- Current unrestricted state license as a Nurse Practitioner or Physician Associate in Louisiana
- Current BLS required, ACLS certification preferred
- Prescriptive authority and DEA
- Valid driver's license with a clean driving record
- Ability to walk up and down several flights of stairs easily
- Maintain all required licenses and certifications, complete all required continuing education courses
- Eligibility to work in the US without Company sponsorship for a work visa or employment-based green card
Special Considerations:
- Comfortable with unplanned care in a nonclinical environment
- Willing to work in patients' homes
- Complete all required mandatory company learning models in a timely manner
- Monthly scheduling will require flexibility and weekend and holiday hours
