Key Responsibilities:
- Conduct comprehensive admission assessments to establish a clear understanding of the patient's condition.
- Document thorough information that includes the care plan, interventions, and outcomes, reflecting the patient's physical, mental, and emotional well-being. Perform audits on documentation from three different patient charts annually.
- Complete transfer and discharge documentation, assessing the patient's needs to identify the preferred level of care.
- Adhere to the five rights of medication administration, clearly communicating to patients and their families the medications being administered.
- Evaluate the patient prior to medication administration, noting any allergies, parameters, potential side effects, and effectiveness, intervening as necessary.
- Manage pain based on patient feedback and assessment findings, reassessing and documenting responses in accordance with hospital standards. Identify high-risk medications and utilize medication references as required.
- Assess patients for potential safety risks upon admission and adhere to hospital policies regarding patient safety, initiating necessary interventions.
- Evaluate the learning needs of patients and their families, planning and delivering appropriate education using effective teaching methods. Document responses to the education provided.
- Implement the five rights of delegation, coordinating and supervising nursing care delivery, and maintaining accountability for delegated tasks.
- Assume the responsibilities of charge nurse, demonstrating problem-solving, decision-making, organizational, and prioritization skills.
- Address unit staffing needs proactively in relation to patient acuity and census, assisting in the orientation and training of new nursing staff.
- Assess the patient's skin condition upon admission, during each shift, and at discharge, noting any changes in condition.
- Initiate preventive skin care measures for patients at risk of skin breakdown, staging pressure ulcers according to established guidelines.
- Comply with Nursing Standards of Practice, including assessment, care planning, interventions, and evaluation of care plans.
- Practice according to unit-specific standards and conduct walking rounds for shift reports and introductory rounds with assigned patients.
- Fulfill all compliance responsibilities associated with the position.
- Perform additional duties as assigned.
