Position Summary
Under the supervision of a Physical Therapist (PT), the Rehabilitation Therapy Assistant (RTA) provides client services as delegated by the PT.

Key Responsibilities
- Assist in evaluating and developing a rehabilitative care plan and conducting periodic re-evaluations as necessary.
- Participate in preparing clinical and daily progress notes, which are reviewed by the PT and submitted according to company policy.
- Perform routine strengthening and therapeutic exercises as directed, and assist patients with self-help devices.
- Collaborate with the PT to instruct clients and their families, assisting in teaching exercise programs.
- Identify potential ineffective drug therapies or adverse reactions, significant side effects, drug allergies, and contraindicated medications, reporting these to the PT.
- Observe and document activities and findings in clinical records, reporting any adverse reactions to treatment to the PT and physician.
- Consult with the PT regarding the outcomes of home visits and report physical findings.
- Instruct other health team members, including Home Health Aides and family members, on specific phases of physical therapy to facilitate client care.
- Write daily progress notes on patients and submit them according to company policy, preparing periodic summary reports as needed.
- Participate in discharge activities with staff, clients, and physicians, assisting the Physical Therapist in completing the Physical Therapy Discharge Summary.
- Confirm visit schedules weekly with the Supervisor/Director of Nursing to coordinate agency visits.
- Assume additional duties as deemed appropriate by the PT and designated by Registered Nurse Case Management and Nursing Supervisor.

Qualifications
- Graduate of an accredited Physical Therapy Assistant program or equivalent education.
- Licensed to practice as a PTA in the State of Florida.
- Current BLS Certification.
- Licensed driver with a reliable automobile insured per organizational requirements.
- Minimum of one year’s experience in home healthcare preferred.
- Experience with electronic medical records is preferred but not essential.
- Must meet all qualifications as stated within the state practice act and maintain current certification in cardiopulmonary resuscitation.
- Must exhibit good emotional health and be able to physically tolerate standing, stooping, bending, and heavy lifting; must comply with all agency regulations.
- Must be self-directed, able to work with minimal supervision, and possess strong observational, judgment, and communication skills.
