Position Overview
The RN Patient Navigator serves as a healthcare professional with specialized knowledge in the diagnosis and management of various orthopedic conditions. This role is pivotal in guiding patients through the healthcare continuum at The Hospitals of Providence - Sierra Campus, ensuring efficient transitions from diagnosis to treatment.
Key Responsibilities
- Facilitate patient navigation through the healthcare system, ensuring timely access to necessary services.
- Collaborate with medical personnel to deliver personalized care tailored to individual patient needs.
- Address educational and emotional requirements of patients and their families, enhancing their understanding of the treatment process.
- Collect and analyze outcome data to assess the effectiveness of navigation services.
Qualifications
Educational Background
- Graduate from an accredited healthcare-related program (e.g., RN, Health Educator, Social Work).
Licenses and Certifications
- Active RN License.
- Current Basic Life Support (BLS) certification for healthcare providers.
Essential Skills and Experience
- A minimum of 2 years of nursing experience.
- Experience in patient navigation is preferred.
- Proficient computer skills.
