Job Description

Job Title: DAC Peer Support Specialist

Job Summary: We are seeking a highly motivated and experienced DAC Peer Support Specialist to join our team at Vaya Health. The successful candidate will be responsible for providing support and coordination to individuals with serious mental illness (SMI) who are being released from a DAC facility.

Key Responsibilities:

  • Coordinate with key providers and stakeholders to ensure timely and thorough documentation of all efforts
  • Ensure that services for the individual are coordinated across the Vaya Health system and with other systems, including primary care
  • Provide support and direction for the individual and their treatment team to ensure quality service and community integration
  • Collaborate with individuals, families, providers, and community stakeholders to create solutions to issues and concerns

Requirements:

  • High school diploma or GED required and at least two years of progressive experience providing similar services to the populations served (MH/SU/IDD)
  • Must have a North Carolina Certified Peer Support Specialist (NCPSS) Certification

Preferred Qualifications:

  • Experience with DAC populations is preferred

Working Conditions:

  • This is a mobile position with work done in a variety of locations
  • Employee is required to have and maintain high-speed internet access in their home residence as a condition of employment
