Job Summary

We are seeking a highly skilled Behavioral Health Therapist to join our team at Mid Michigan Health. As a key member of our primary care team, you will play a vital role in providing high-quality behavioral health services to our patients.

Key Responsibilities
  • Assess and manage patients with medical and behavioral health problems, working closely with our primary care providers to develop comprehensive treatment plans.
  • Provide brief behavioral interventions using evidence-based techniques, such as behavioral activation, problem-solving treatment, and motivational interviewing.
  • Collaborate with our primary care team to identify and address the needs of patients with chronic emotional and physical health problems.
  • Develop and implement population health programs to promote patient self-management and improve health outcomes.
  • Document patient progress and treatment recommendations in our Electronic Health Record (EHR) system.
Requirements
  • Master's degree in Social Work (MSW) or related field.
  • Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan.
  • Experience working in primary care settings, preferably in an integrated behavioral health role.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
