Job Description

As a Full-Time RN Case Manager at Gentiva, you will be responsible for providing comprehensive patient care and support, working closely with physicians, caregivers, and other healthcare professionals to ensure timely and effective delivery of care. Your primary responsibilities will include:

  • Patient Assessment and Care Planning: Conduct thorough assessments of patient needs and develop personalized care plans that address physical, emotional, and spiritual concerns.
  • Care Coordination: Collaborate with healthcare teams to ensure timely and effective delivery of care, including medication management, wound care, and symptom management.
  • Communication and Education: Educate patients and families on disease progression, treatment options, and end-of-life care, promoting informed decision-making and empowering patients to take an active role in their care.
  • Case Management: Manage patient caseloads, ensuring efficient use of resources and minimizing hospitalizations and readmissions.
  • Interdisciplinary Collaboration: Work effectively with interdisciplinary teams to develop and implement comprehensive care plans, incorporating input from physicians, social workers, chaplains, and other healthcare professionals.

Requirements and Qualifications

  • Nursing Experience: Minimum 1 year of experience in hospice, home health, or related field, with strong clinical skills and ability to work independently.
  • RN License: Current RN license in the state of employment, with eligibility to obtain if not currently licensed.
  • Education and Training: Bachelor's degree in Nursing required; Master's degree preferred.
  • Additional Skills: Strong communication and interpersonal skills, with ability to work effectively with diverse populations and healthcare teams.
