We are seeking a highly skilled and compassionate Care Management Resource Coordinator to join our team at Phoenix Children's Hospital. As a key member of our Care Management Department, you will play a vital role in supporting the efforts of Social Work, Case Management, and Utilization Management professionals.
Key Responsibilities- Support Social Work activities by receiving, evaluating, and fulfilling requests for local transportation vouchers, meal cards, and other amenities.
- Facilitate requests for out-of-area transportation, including air travel and lodging, and ensure timely and safe transportation for patients and families.
- Collaborate with patients, families, and healthcare organizations to coordinate post-discharge needs, including scheduling services and coordinating availability of equipment and supplies.
- Monitor and determine bed availability at rehab facilities, other acute care hospitals, long-term care facilities, mental health facilities, and other organizations, and ensure timely and safe transfers.
- Provide excellent customer service by responding to emails and phone calls in a timely and professional manner, and ensuring clear communication with all internal and external customers.
- High school diploma or equivalent required; associate's or bachelor's degree preferred.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Strong organizational and time management skills, with the ability to maintain accurate and complete records.
- A dynamic and supportive work environment with opportunities for professional growth and development.
